Intake Process

Eligibility Criteria

The service user:

Must be between 18-65 years of age. Special consideration may be given to clients over 65 years who have met the criteria for admission.
Must consent to treatment.
Must have a history of a mental health condition, and have some insight into their mental health needs.
Should be able to participate in multidisciplinary activity planning and carry out activities of daily living with minimal supervision.
Must be willing to continue recommended psychiatric and medical treatment.
Must be willing to share a gender-neutral multi-cultural community setting for the duration of their admission.
Should be willing to engage in all clinical and rehabilitative programs. These include but are not limited to clinical reviews, psychotherapeutic, occupational therapy, vocational, recreational and educational programs.
Must understand and accept the terms and conditions of residency at PRC and will be required to abide by PRC policies and procedures.
Must agree to refrain from illicit drugs and to abide by the drug and alcohol policy of PRC, for the duration of their residency. All service users may be required to do a mandatory drug screen on admission and randomly throughout their admission to ensure compliance with the PRC drug/alcohol policy.

PRC cannot provide an appropriate service if –

Service user has not attained the age of 18 years old.
Service users are in an acute phase of mental ill health.
Service users are exhibiting persistent aggressive or threatening behaviour.
Service users have a repetitive history of fire setting.
Service users have severe organic brain syndrome, neurocognitive, neurodegenerative or other medical conditions.
Service users pose a risk of harm to self or others including a risk of sexual harm.

NOTE:

No emergency admissions will be accepted by the facility.
The criteria and exclusions are not exhaustive but are a general guide to our admission approach.
All admissions are mixed of voluntary with some involuntary under the Mental Health Act. Persons being treated on assisted outpatient treatment order (AOTO) can be considered if they consent to be treated at Poinciana, demonstrate willingness to continue psychiatric interventions, and meet the eligibility criteria.
PRC is not a designated place of safety.

Poinciana
Rehabilitation Centre

Poinciana Rehabilitation Centre (PRC), located in East End, Grand Cayman, Cayman Islands is a tertiary care, residential mental health facility under the Ministry of Health, Enviroment and Sustainability. It was established for residents in the Cayman Islands that have serious and persistent mental illness to be cared for holistically in a safe and secure environment reflecting a therapeutic approach to care. The services form a part of the continuum of mental health care in the Cayman Islands. The main goals are to provide rehabilitation services and to reintegrate residents back to their communities.

SERVICES OFFERED

Clinical Interventions

Initial and ongoing psychiatric assessment, monitoring and care. This includes reviews from our on-site psychiatrist, medical practitioners, dietician and the provision of 24-hour specialised psychiatric nursing care.

Psychological Interventions

Psychotherapeutic assessments and interventions, provided by clinical and counselling psychologists, will be in the form of individual, family and group therapies.

Rehabilitation Services

Will mainly involve recreational and occupational therapy services and vocational training. These are aimed at improving the resident’s emotional wellbeing and equipping residents with skills that can make them employable.

Social Interventions

Providing social support to aid residents’ reintegration with their families and communities. Services will include the implementation of support groups and day programmes for internal residents and external clients. Services will be provided by licensed social workers who will work with the individual residents, families and communities.

Mental Health Promotion, Public Awareness & Education

Promoting mental health awareness in the community and reducing stigma and discrimination, and improving the residents’ compliance to the care process.

CLINICAL SERVICE

The Clinical Service is one of the most significant aspect of the services at PRC. A multidisciplinary team approach is utilized to provide holistic care guided by the biopsychosocial model. The team is responsible for overseeing the provision of clinical services and ensures the delivery of high-quality therapeutic care. The multidisciplinary team consists of medical practitioners, nursing and allied healthcare workers

Assessment and Treatment Process

Assessment will be an ongoing process. Conducting a full assessment for the incoming residents are necessary even though they will be coming from another treatment program. This is necessary to ensure the accuracy of the admitting diagnosis or to conceptualize new and or additional diagnoses and to formulate treatment plan for the best possible outcome. The clinical team will employ keen observation, interview, examination and critical thinking skills to formulate a diagnosis and treatment plan utilizing the Biopsychosocial Model and Case Formulation.

The treatment plan will incorporate the holistic approach and will include the necessary referrals for psychosocial interventions and nutritional interventions.

Review of Residents

All residents will be reviewed by the psychiatrist as necessary weekly after the initial assessment. Reviews will also be done in cases of emergency or as the need arises. Reviews and updated treatment plans should be documented in the resident’s Electronic Health Record. Reviews by the physician and nutritionist will be done as needed.

Diagnostics Investigations

Medication used to treat psychiatric conditions can have an impact of the physical health of individuals. It is of importance for all residents to have diagnostic investigations performed as a baseline to determine their physical status upon admission as well as to monitor underlying health conditions.

Psychotherapeutic Interventions

Psychotherapy will be done primarily by the psychologist. Each resident will be individually assessed and a treatment plan tailored to meet the identified need/s. Interventions may be in the form of individual, family and group sessions and other related intervention.

Social Interventions

Social interventions are necessary to provide social benefits and help to build human capital and providing socioeconomic benefits for vulnerable groups. Programs will be designed to help to raise the standard of living and earning capacity for the residents at PRC. The Programs will include helping residents getting financial assistance, housing assistance, family and community investigations and resources necessary to facilitate their reintegration and to help them to live independently. Social workers will be responsible for conducting the initial assessment for each resident to determine their needs and the level of support needed. They will also be responsible for doing follow-up visits following discharge, to ensure that residents are effectively reintegrated.

Discharge

Collaborated discharge planning will commence upon admission and will be completed prior to the discharge of each resident. This will include any social investigations and networking that are necessary to help residents re-integrate with families and communities. It will also include the necessary education on medication management and the importance of keeping appointment for follow-up care.

Follow-up

Follow-up reviews may be facilitated through the multi-disciplinary team at Poinciana and in collaboration with the HSA Behavioral Health Unit. Members of the clinical team will be responsible to ensure that residents receive follow-up care interventions as necessary when discharge. A detailed follow-up plan outlining the current diagnosis, medications and other needed intervention/s, will be provided by the treatment provider or caregiver.

REHABILITATION SERVICES

Mental health problems and disorders reduce the ability of persons affected to function at the level they previously did prior to becoming ill. Some individuals have lost the skills they previously had and have become dependent on family, communities and the society at large to assist them in performing tasks and providing for their needs. Rehabilitation programs at PRC are geared towards helping these individuals improve their capacity to function independently, regaining previously lost skills and gain additional skills or maintain their level of functioning.

All residents will be involved in one or more of these rehabilitation Programs.

Rehabilitation Programs will be conducted at different locations at the Centre as well as in external entities that will facility the residents.

Behavioral and Psychosocial Interventions

Behavioral and psychosocial interventions are an important aspect of the recovery process for the residents. Residents will be engaged in psychotherapeutic activities by the Psychologist and other team members, utilizing different approaches outlined in the psychological program.

Social interventions will be the main responsibility of the Social Workers which will be geared towards helping residents with basic life and social skills. Additionally, individual, family and community assessments, financial and placements assistance will be implemented to help with improving residents’ independence and re-integration in communities.

Occupational Therapy

Occupational therapy will be used to equip residents with skills that can improve their income generating capacity and social skills. This would help to give residents a sense of hope, improving self-esteem and assisting them to contribute to their personal wellbeing. The programs will include but not limited craft making, cooking, furniture making, agriculture activities. Others will be added based on the needs of the residents.

Vocational Skills Training

This is aligned to the occupational therapy program but has more formal theoretical content that will be undertaken by external persons and/or agencies. This will be in the format of impart educational and skills in a variety of areas that may lead to certification and increasing their potential of becoming employable.

Recreational and Music Therapy

“Recreational therapy uses leisure activities to help people with specific health conditions improve their skills, abilities, overall health, and emotional well-being.” (R. J. Stanborough, 2020). It includes a variety of activities from arts, physical activities, sports and games, dance, gardening, drama, animal interaction. The use of this form of therapy have been found to be very beneficial in supporting the emotional wellbeing of individuals.

Basic Life and Social Skills Training

Basic life and Social skills training involve illness management Programs aimed at achieving and maintaining recovery. These include activities that promotes medication management to improve compliance, increases financial awareness, communication, domestic, personal self-care and community living skills.

Support Groups

Support groups are often formed to allow persons to come together to share challenges, experiences, solutions to problems they may have. It gives persons a sense of belonging and helps them realize that they are not alone, there are others that share similar issues.
